Transition Coordinator in our Youth and Young Adult Services
(YAYAS) Programs, which have been featured in over a dozen books

The Transition Coordinator serves as the primary transition
facilitator for youth and young adults who are discharging from the
Young Adult Residential and Transitional Living Program. They
provide the coordination of mental health treatment, housing,
benefits, and linkage to other services and resources that will be
needed post-discharge. They also assist in skill-building through
coaching and one-on-one work with young adults as they prepare for
the transition. ESSENTIAL DUTIES & RESPONSIBILITIES Provides direct
service to members through transition, including case management,
crisis intervention, and psycho-social rehabilitation services.
Develops and implements the transition/discharge plan, in
cooperation with other members of the transition/treatment team.
Connects member with community and Thresholds resources, including
appropriate housing, mental health and psychiatric services,
physical health services, public assistance benefits, resources for
social/leisure activities, and developing connections to family and
others. Provides direct assistance to help members move, obtain
household items, and explore their new community at time of
discharge from residential program. Maintains contact with members
post-discharge from the residence to provide continuity of care and
track outcomes. Primary contact between agency, families, and other
collateral resources to assist with members' transition goals and
needs. Performs and participates in crisis intervention using
Therapeutic Crisis Intervention (TCI). Performs and supervises
physical management when necessary, according to Thresholds policy.
Thresholds provides and requires training in TCI. Attends all
transition-related court appearances, DCFS meetings, agency
staffings, team meetings, treatment team meetings, and other
meetings as assigned. Ensures all Thresholds procedures regarding
the handling of members' funds are followed and meet the highest
standards. Completes all assigned paperwork in a timely fashion.
Produces Medicaid compliant service notes. Completes reports for
Thresholds and other stakeholders when required. Facilitates group
programming related to transition in both the YAP residential
program and the REACH program. Teaches financial literacy
curriculum to members. May be required to work flexible schedules
and be available to work on weekends or evenings as needed. May be
required to be on call. EDUCATION & EXPERIENCE Bachelor's degree in
psychology, social work, or a related field required. Master's
degree preferred. Previous experience working with young adults
strongly preferred. Previous experience working with housing
assistance programs and/or public assistance benefits systems
strongly preferred. REQUIREMENTS Daily access to a personal vehicle
Valid driver’s license Willingness to provide transportation for
members of the program Must obtain and maintain liability car
insurance of: $100,000 per person bodily injury $300,000 per
