Job Description:
Encompass Health is seeking a Certified Case Manager to join our
team, dedicated to revolutionizing patient care. Our Case Managers
play a pivotal role in managing interdisciplinary care plans and
ensuring the seamless transition of patients from healthcare
facilities to home care. Working closely with patients, families,
and healthcare teams, you will oversee the assessment, planning,
facilitation, and advocacy for options and services to meet an
individual’s health needs through communications and available
resources to promote quality, cost-effective outcomes. Assess new
patients and determine the appropriate level of care and the
necessary resources. Develop patient-specific care plans in
collaboration with healthcare professionals. Act as a liaison
between patients, family members, and various health care
providers. Monitor the implementation of care plans and adjust as
necessary to achieve medical and health goals. Educate patients and
their families about the care process and empower them with the
knowledge needed for recovery. Provide resources and support to
patients transitioning from hospital care to home or rehabilitation
facilities. Strong interpersonal and communication skills Ability
to manage multiple case loads effectively Thorough knowledge of
healthcare protocols and best practices Proficiency in
documentation and care management software Medical Insurance Dental
Insurance Vision Insurance Life Insurance 401k Certified Case
Manager (CCM) credential is required. Additional certification in
Healthcare Management or related field is a plus.
