About Us Our organization has been a pioneer in establishing the
first national in-home clinical network, currently functioning as
an independent provider of extensive health assessments for
individuals nationwide. We are dedicated to delivering exceptional,
personalized care that enhances health, reduces costs, and empowers
individuals to lead fulfilling lives. With a strong foundation in
clinical assessment and care management, our nationwide network of
clinicians transcends traditional barriers by meeting members in
their own environments. We assist older adults and other at-risk
populations in achieving improved health outcomes and identifying
chronic conditions that might otherwise remain undetected. Joining
our team offers you the chance to utilize your skills and
compassion to significantly influence the health and well-being of
others. Be part of our mission and enjoy competitive pay along with
flexible scheduling while making a positive impact in the
community! Responsibilities About the Role During visits that may
last up to one hour, our providers assess and observe a member’s
current health status, medical history, medication adherence,
social circumstances, and other potential risks. This approach
provides invaluable insights into a member’s overall health that
are often missed during standard office visits. The Matrix
Comprehensive Health Assessment (CHA) enhances care quality and
allows us to address multiple care gaps in a single visit. Our
providers are responsible for: Conducting assessments for adults
and geriatrics, including medical history, diagnosis and treatment,
health education, physician referrals, case management referrals,
follow-up, and thorough documentation in accordance with our
guidelines and protocols. Administering and ordering necessary
point-of-care and lab-based tests. Collaborating with physicians,
case managers, social workers, family members, key caregivers, and
other medical personnel as appropriate. Working alongside Primary
Care Physicians (PCPs) to educate patients and ensure necessary
follow-ups. Providing services at the member’s residence, which may
include their homes or Skilled Nursing Facilities. Benefits of
Working with Us Flexibility – Schedule visits based on YOUR
availability. Assessments are conducted from Sunday to Saturday,
between 8 am and 7 pm. You can choose to work during the day,
evening, or weekends according to your preferences. Competitive Pay
– Your earnings are based on the number of assessments completed.
Increase your income potential by being available for more visits
and traveling to see members outside your home area. Those
averaging 20 to 100 visits monthly can earn between $1,800 and
$11,500 per month, depending on the per assessment rate of $90 to
$115 based on your state of residence. Benefits – Including mileage
reimbursement, a 401(k) plan, employee referral bonuses, and
limited voluntary benefits. Education and Support – We invest in
your growth! Along with comprehensive onboarding training, you will
be assigned a preceptor, receive personalized feedback, and have
free access to continuing education opportunities. Additional

neighboring states or others you may wish to visit while conducting
health assessments. Full-time Opportunities – Our PRN providers
