Job Description Perm Medical Director – Geriatric PACE Program Location: Mishawaka, Indiana Schedule: Full-time, weekday position with some regional travel An established PACE (Program of All-Inclusive Care for the Elderly) center in Mishawaka, IN is seeking a Board Certified Internal Medicine or Family Medicine physician with Geriatric Medicine training or CAQ to serve as Medical Director. This is a permanent, leadership-focused role guiding clinical care and interdisciplinary coordination for frail seniors in a highly collaborative environment. Key Responsibilities - Lead and supervise all medical aspects of participant care - Oversee and develop medical policies, QAPI program, and care standards - Supervise PCPs and APPs - Provide direct care: H&Ps, chronic disease management, reassessments, and house calls - Participate in IDT meetings and support care planning - Engage in community outreach and leadership activities - Participate in rotating on-call coverage Candidate Requirements - Board Certified in Internal Medicine or Family Medicine – Required - Board Certified in Geriatrics or CAQ in Geriatric Medicine – Highly Preferred - IN license (or willingness to obtain) – Required - Minimum 1 year of experience with elderly or frail populations – Required - Prior supervisory experience – Required - BLS, CDS, DEA (or willingness to obtain prior to start) – Required - Local and permanent candidates only (no Locums or visa sponsorship) Compensation & Benefits - Base Salary: $231,000–$280,000 (based on experience) - Comprehensive benefits: Medical, Dental, Vision - 403(b) retirement plan with match - Paid Time Off CME allowance - Malpractice coverage included - Tuition reimbursement available - Professional dues and fees covered About the Community Located just 80 miles from Chicago, Mishawaka/South Bend offers a vibrant community with a low cost of living, excellent schools, four-season recreation, and proximity to Lake Michigan and the University of Notre Dame.
