Case Manager and be part of a multi-disciplinary team delivering
recovery-oriented services to clients with dual diagnoses (mental
illness and developmental disabilities), and sometimes co-occurring
physical or substance abuse issues.What You'll Do:Conduct Mental
Health Assessments and create personalized treatment plans using
the IM/CANS tool.Provide counseling/therapy tailored to each
client's unique needs.Oversee SSI & LINK benefits submissions and
approvals.Connect clients to crucial services like housing,
education, and vocational programs.Lead individual and group
skill-building activities in the client's natural environment
(home, school, community).Collaborate with families, guardians, and

contact.Requirements:QUALIFICATIONS:Bachelor's degree in social
work, counseling, psychology or related field. 1-3 years of
experience in working within the social service arena, including
internships. Must be able to work with diverse populations with
multi-faceted needs. Experience with mental health and I/DD
populations and knowledge of Medicaid standards. Bilingual in
Spanish or Polish is required. Must be at least 21 years of age,
possess a valid driver's license or state ID, be approved to drive
agency vehicles through agency insurance, pass a criminal
background check. Must have excellent written and oral
communication skills. Compensation & Benefits:Base Salary: $50,000
per year + additional quarterly productivity bonuses Paid Time Off:
11 Vacation days, 12 paid Holidays, 3 sick days, 3 personal
