Job Description:
Seeking a dedicated REGISTERED NURSE or LPN -----" HOME HEALTH
to uphold core values of compassion, accountability, collaboration,
foresight, and joy.
Responsibilities include assessment, planning, implementation, and
evaluation of patient care based on evidence-based practices.
Full-time, day shift with on-call duties 2-3 nights/month and every
5th weekend.
Qualifications: active Wisconsin RN or LPN license, relevant
experience, BLS certification within 90 days, and specialty
certifications as needed.
